Senior Social Worker required for the Hospital Team The Hospital team facilitates discharges using D2A from Kingston Hospital and all Out of Borough Hospitals. Applicants must have recent sound knowledge and experience of Hospital Discharge and good IT skills Knowledge of LAS/ IAS and Google an advantage. Will be required to be based one day a week at Kingston Hospital To also be willing to travel to Kingston when required to complete assessments- CAA/ MCAs etc
