We are looking for committed and proactive Preparation for Adulthood (PfA) Officers to join our SEN service. You will be at the heart of supporting young people as they prepare to move from school or college into adult life. Your role will focus on helping them plan, ensuring their EHCPs reflect their aspirations and that the right support is in place to make them a reality. You will work closely with young people, their families, schools, colleges, social care and health professionals. You will attend and contribute to annual reviews from Year 9 onwards, making sure that PfA outcomes – such as employment, independent living, community participation and good health – are clearly embedded in each plan. You will promote access to opportunities such as supported internships, apprenticeships and training, and work with professionals to ensure young people can move smoothly into adult services where needed. To succeed in this role, you will be an excellent communicator who can explain complex processes clearly and sensitively. You will have empathy, patience and a strong belief in the potential of every young person. Strong organisational skills and the ability to manage competing priorities are essential, as is the confidence to work across multiple agencies. Experience of transition planning and knowledge of the SEND Code of Practice Knowledge is essential. The ability to use Synergy is desirable.
